ABSTRACT

La amiloidosis siempre ha representado un desafío diagnóstico. En el año 2020, el Grupo de Estudio de Amiloidosis (GEA), confeccionó la Guía de Práctica Clínica para el Diagnóstico de Amiloidosis. Nuevas líneas de investigación se han desarrollado posteriormente. Esta revisión narrativa tiene como intención explorar el estado del arte en el diagnóstico de la amiloidosis. En pacientes con amiloidosis se recomienda la tipificación de la proteína mediante espectrometría de masa, técnica de difícil ejecución por requerir de microdisectores láser para la preparación de la muestra. Algunas publicaciones recientes proponen otros métodos para obtener la muestra de amiloide que se va a analizar, permitiendo prescindir de la microdisección. Por otra parte, en pacientes con Amiloidosis ATTR confirmada, la recomendación de secuenciar el gen amiloidogénico se encontraba destinada a los casos sospechosos de ATTR hereditaria (ATTRv,), pero actualmente esta se ha extendido a todos los pacientes sin importar la edad. En lo que respecta a los estudios complementarios orientados al diagnóstico de compromiso cardíaco, se ha propuesto el uso de la inteligencia artificial para su interpretación, permitiendo la detección temprana de la enfermedad y el correcto diagnóstico diferencial. Para el diagnóstico de neuropatía, las últimas publicaciones proponen el uso de la cadena ligera de neurofilamento sérica, que también podría resultar un indicador útil para seguimiento. Finalmente, con referencia a la amiloidosis AL, la comunidad científica se encuentra interesada en definir qué características determinan el carácter amiloidogénico de las cadenas livianas. La N-glicosilación de dichas proteínas impresiona ser uno de los determinantes en cuestión. (AU)


Amyloidosis has always represented a diagnostic challenge. In 2020, the Amyloidosis Study Group (ASG) developed the "Clinical Practice Guideline for the Diagnosis of Amyloidosis". New lines of research have subsequently emerged. This narrative review aims to explore the state of the art in the diagnosis of amyloidosis diagnosis. In patients with amyloidosis, protein typing by mass spectrometry is recommended, a technique hard to perform because it requires laser microdissection for sample preparation. Recent publications propose other methods to obtain the amyloid sample to be analyzed, making it possible to dispense with microdissection. On the other hand, in patients with confirmed TTR amyloidosis (aTTR), the recommendation to sequence the amyloidogenic gene was intended for suspected cases of hereditary aTTR but has now been extended to all patients regardless of age. ABSTRACT

La amiloidosis AL es una enfermedad debida al depósito, en órganos y tejidos, de fibrillas formadas por cadenas livianas producidas de forma patológica por plasmocitos clonales. Su tratamiento actualmente está orientado a erradicar el clon de células plasmáticas; este históricamente se extrapoló de tratamientos disponibles y estudiados para otras discrasias sanguíneas. En el año 2020, el Grupo de Estudio de Amiloidosis (GEA) confeccionó distintas guías de práctica clínica para el tratamiento de la amiloidosis AL. Desde entonces se han publicado ensayos clínicos que arrojan contundencia al conocimiento disponible hasta el momento, y están en desarrollo nuevas líneas de investigación que robustecen y estimulan el estudio en el área. En esta revisión se realiza una actualización de las guías existentes en lo que respecta al tratamiento de la amiloidosis por cadenas livianas.Como evidencia de relevancia, en el último año estuvieron disponibles resultados de ensayos clínicos que respaldan el uso de esquemas basados en daratumumab (un anticuerpo monoclonal anti-CD38+) para pacientes con diagnóstico reciente de amiloidosis AL como primera línea. Además, para el tratamiento de la amiloidosis AL refractaria o recaída, la disponibilidad de bibliografía respaldatoria es escasa y extrapolada del tratamiento del mieloma múltiple; sin embargo, actualmente existe evidencia de calidad para recomendar el uso de ixazomib, un inhibidor de proteosoma reversible por vía oral disponible en la Argentina desde 2020. Por último, se mencionan algunas líneas de investigación con otros anticuerpos monoclonales y terapéuticas basadas en el uso de CAR-T cells. (AU)


AL amyloidosis is a disease caused by the deposit in different organs and tissues of protein fibrils formed by light chains synthetized by pathological clonal plasma cells. Its treatment is currently aimed at eradicating this plasma cell clone and it has been historically extrapolated from available and validated treatments for other blood dyscrasias. In 2020, the Amyloidosis Study Group prepared different clinical practice guidelines for the treatment of AL amyloidosis.Since then, clinical trials have been published that confirm and strengthen the knowledge available up to now, and new lines of research are being developed that stimulate study in the area. In this review, an update of the existing guidelines regarding the treatment of AL amyloidosis is made. As relevant evidence, in the last year, results of clinical trials have been made available that support the use of regimens based on Daratumumab (an anti-CD38+ monoclonal antibody) for patients with newly diagnosed AL amyloidosis as first line therapy. In addition, for the treatment of refractory or relapsed AL amyloidosis, where the availability of supporting literature is scant and extrapolated from the treatment of multiple myeloma, there is currently quality evidence to recommend the use of ixazomib, an oral reversible proteasome inhibitor, only available in Argentina since 2020. Finally, some research lines exploring the efficacy of other monoclonal antibodies and therapeutic experiments based on the use of CAR-T cells are mentioned. (AU)

Introdução: Materiais educativos sobre cuidados com a voz dão apoio no atendimento clínico e prevenção de disfonia, entretanto, são escassos. Portanto, este estudo objetivou elaborar e avaliar um guia sobre saúde vocal infantil para pais e crianças. Descrição: A elaboração do guia abrangeu: Levantamento Bibliográfico nas bases de dados Biblioteca Virtual em Saúde (BVS) e Público/editora MEDLINE (PubMed); Escrita dos temas; Organização/escrita dos conteúdos/referências; Seleção de imagens. Estruturação do guia: Produção da voz; Sinais/sintomas mais comuns de alterações vocais; Causas dos distúrbios vocais infantis; Consequências do distúrbio vocal infantil; Profissionais envolvidos no diagnóstico e tratamento; Mito e Verdade sobre voz; Como prevenir o distúrbio vocal infantil; Atividades amigas da voz. A avaliação foi em grupo focal, via Google Meet, com três juízes mestrandos em Fonoaudiologia, que discutiram qualitativamente estética, conteúdo e organização. A discussão foi coordenada pela coorientadora e as indicações, realizadas por consenso entre os juízes: Estética - ajustar local das referências, elaborar jogo de trilha, uniformizar desenhos/cores e criar mascote; Conteúdo - material relevante, diminuir textos, adequar a linguagem para crianças, usar links/QR-Code para informações extras e acrescentar orientações para professores; Organização - tópicos em ordem hierárquica, conteúdo relacionado ao tema e separar assuntos por capítulos. Considerações Finais: Foram apontadas mudanças, porém, os juízes ressaltaram a importância deste material na clínica fonoaudiológica e na promoção de saúde vocal. O grupo focal foi importante para a primeira avaliação do guia. (AU)


Introduction: Educational materials on voice care support in clinical care and dysphonia prevention, however, are scarce. Therefore, this study aimed to elaborate and evaluate a guide on child vocal health for parents and children. Description: The elaboration of the guide covered: Bibliographic Survey in the databases Biblioteca Virtual em Saúde (BVS) and MEDLINE Public/Publisher (Pubmed); Themes writing; Organization/writing of the contents/references; Selection of images. Structure of the guide: Voice production; Most common signs/symptoms of vocal disorders; Causes of voice disorders in childhood; Consequences of voice disorders in childhood; Professionals involved in diagnosis and treatment; Myth and Truth about voice; How to prevent vocal disorder in childhood; Voice-friendly activities. The evaluation was in a focus group, via Google Meet, with three Master Judges in Speech Therapy, who discussed qualitatively aesthetics, content and organization. The discussion was coordinated by the co-supervisor and the following recommendations were consensus among the judges: Aesthetics - adjust location of references, elaborate track game, standardize drawings/colors and, create mascot; Content -relevant material, decrease texts, tailor language for children, use/QR-Code links for extra information and, add guidance for teachers; Organization - topics in hierarchical order, content according to theme and separate subjects by chapters. Final Considerations: Improvements were pointed out, however, the judges emphasized the importance of this material in the speech therapy clinic and vocal health promotion. The focus group was important for the guide's first evaluation. (AU)

La discusión fue conducida por la coordinadora y las indicaciones, realizadas por consenso entre los jueces: Estética - ajustar lugar de las referencias, elaborar juego de pista, uniformizar dibujos/colores y crear mascota; Contenido - material relevante, disminuir textos, adecuar el lenguaje para niños, usar links/QR-Code para informaciones extras y añadir orientaciones para profesores; Organización - tópicos en orden jerárquico, contenido relacionado al tema y separar asuntos por capítulos. Consideraciones Finales: Se señalaron cambios, sin embargo, los jueces resaltaron la importancia de este material en la clínica fonoaudiológica y en la promoción de salud vocal. Las recomendaciones presentadas en esta guía fueron discutidas y aceptadas en un panel multidisciplinario de profesionales, con participación de pacientes, sociedades médicas y expertos en el tema, del Sistema Nacional Integrado de Salud (SNIS), quienes contribuyeron a definir la fuerza de las recomendaciones, validando cada una de ellas con la calidad de la evidencia y fuerza de la recomendación siguiendo el sistema GRADE .Ponemos este documento a la disposición y alcance de los profesionales de la salud para su uso en la práctica diaria ya que debe ser prioridad, ofrecer a las pacientes una detección temprana de las lesiones premalignas del cérvix y brindar un tratamiento oportuno


The recommendations presented in this guide were discussed and accepted in a multidisciplinary panel of professionals, with the participation of patients, medical societies and experts in the subject, of the National Integrated Health System (SNIS), who contributed to defining the strength of the recommendations, validating each of them with the quality of the evidence and strength of the recommendation following the GRADE system. We make this document available and accessible to health professionals for use in daily practice as it should be a priority, offer patients early detection of premalignant lesions of the cervix and provide timely treatment

Objetivo: construir e validar uma escala de verificação da adesão às recomendações das Diretrizes Brasileiras de Ventilação Mecânica por profissionais da saúde. Método: estudo metodológico, conduzido no período entre setembro e dezembro de 2019 em um hospital público com 87 pacientes. Para a validação de conteúdo, adotou-se o Índice de Validação de Conteúdo; para a validade de critério, o Coeficiente de Correlação de Pearson; para a consistência interna, o alfa de Cronbach; e, para a confiabilidade interobservador, o Coeficiente Kappa e o Coeficiente de Correlação Intraclasse. Resultados: a escala identificou uma validade de conteúdo e consistência interna aceitável. A correlação de Pearson indicou uma correlação do escore de adesão com a saturação (r = 0,31; p≤0,005), o escore médio para o observador A e B resultou, respectivamente, em 88,89(±5,23) e 88,86(±5,34), e o intervalo de confiança foi de 0,96. Conclusão: a escala apresentou validade e confiabilidade para verificar a adesão às Diretrizes Brasileiras de Ventilação Mecânica dos profissionais.(AU)


Objective: to construct and validate a scale for verifying adherence to the recommendations of the Brazilian Guidelines for Mechanical Ventilation by healthcare professionals. Method: methodological study, conducted between September and December 2019 in a public hospital with 87 patients. For content validation, the Content Validation Index was adopted; for criterion validity, Pearson's Correlation Coefficient; for internal consistency, Cronbach's alpha; and, for interobserver reliability, the Kappa Coefficient, and the Intraclass Correlation Coefficient. Results: the scale identified acceptable content validity and internal consistency. Pearson's correlation indicated a correlation between adherence score and saturation (r = 0.31; p≤0.005), the average score for observer A and B resulted, respectively, in 88.89(±5.23) and 88.86(±5.34), and the confidence interval was 0.96. Conclusion: the scale showed validity and reliability to verify adherence to the Brazilian Guidelines for Mechanical Ventilation by professionals.(AU)

ABSTRACT

Objetivo: construir e validar as competências profissionais para enfermeiros atuantes em Unidades de Hemodinâmica. Método: trata-se de uma pesquisa metodológica de construção e validação das competências do enfermeiro de hemodinâmica, aprovada pelo Comitê de Ética em Pesquisa sob protocolo nº 67891517.8.0000.5462 e realizada em instituição pública de cardiologia do estado de São Paulo. Na primeira etapa, foi construído o instrumento para identificar as competências do enfermeiro no setor de hemodinâmica a partir de revisão de literatura e observação local; na segunda, o instrumento foi validade por especialistas. Os dados foram analisados pelo Índice de Validade de Conteúdo (IVC) e média ponderada do IVC. Resultados: a validação das competências ocorreu após duas rodadas de avaliação junto aos especialistas, resultando num total de sete competências relativas ao processo de trabalho da Enfermagem, com média ponderada de IVC entre 88,4 e 99,2 e 74 habilidades, as quais foram classificadas quanto ao tipo de competência; 14 itens foram avaliados como básicos, 10 como intermediários, 34 como avançados e 16 como inconclusivos. Conclusão: as competências profissionais para enfermeiros atuantes em Unidades de Hemodinâmica foram construídas e validadas por especialistas, os quais poderão subsidiar novas diretrizes sobre a formação e a educação permanente dos profissionais nessa área.(AU)


Objective: to build and validate professional skills for nurses working in Hemodynamics Units. Method: this is a methodological study for the construction and validation of hemodynamic nurses' skills, approved by the Research Ethics Committee under protocol number 67891517.8.0000.5462 and carried out in a public institution of cardiology in the state of São Paulo. In the first stage, an instrument was built to identify the skills of nurses in the hemodynamics sector based on a literature review and local observation; in the second, the instrument was validated by specialists. Data were analyzed by Content Validity Index (CVI) and CVI weighted average. Results: competency validation took place after two rounds of evaluation with specialists, resulting in a total of seven competencies related to the Nursing work process, with a weighted average CVI between 88.4 and 99.2 and 74 skills, which were classified according to the type of competence; 14 items were assessed as basic, 10 as intermediate, 34 as advanced and 16 as inconclusive. Conclusion: the professional competences for nurses working in Hemodynamics Units were built and validated by specialists, who can subsidize new guidelines on the formation and permanent education of professionals in this area.(AU)

ABSTRACT

O presente texto tem o objetivo de explanar ações desempenhadas por psicólogas(os) trabalhadoras(es) dos Centros de Referência Especializados de Assistência Social (Creas), em situações de violência intrafamiliar, identificadas a partir do estude empírico realizado por mim, psicóloga pesquisadora, também trabalhadora de um Creas. Participaram da pesquisa doze psicólogas(os), trabalhadoras(es) destes centros, em sete municípios do interior do Rio Grande do Sul, onde foram realizadas, presencialmente, as entrevistas. A análise dos dados apontou para uma compreensão metodológica a partir de três dimensões já apontadas na bibliografia, sendo elas: a) Acolhida Inicial, que demonstra que esses profissionais geralmente iniciam suas práticas com uma família ou indivíduo indo ao encontro destes, buscando a vinculação dos mesmos com o serviço; b) Acompanhamento Especializado, onde essas(es) trabalhadoras(es) desenvolvem suas práticas com diversidade e criatividade, a partir de visitas domiciliares, trabalhos com grupos, indivíduos ou famílias, geralmente em conjunto com outros profissionais, principalmente assistentes sociais; c) Articulação com a Rede, onde se identificou um importante movimento para o trabalho em conjunto com outros serviços disponíveis no território. Por fim, as considerações finais indicam que ainda há um caminho a ser trilhado com relação à definição das práticas dos psicólogos no Creas. Porém, há muito que se falar a respeito de práticas que já estão ocorrendo. Assim, tornam-se relevantes as pesquisas acadêmicas nesse contexto, pois ao inserir os profissionais psicólogos trabalhadores da política, eles podem promover uma articulação entre a produção do fazer cotidiano e a reflexão teórica e acadêmica sustentada pelas pesquisas.(AU)


This study aims to explore the activities developed by psychologists from the Centros de Referência de Atenção Especializada (CREAS - Brazilian Specialized Social Assistance Reference Centers), regarding situations of intrafamily violence identified by me, the author, a research psychologist and, CREAS worker. Participants include 12 psychologists who work on such centers from 7 cities in the countryside of Rio Grande do Sul, Brazil, where the interviews were conducted in person. The data analysis pointed toward a methodological comprehension based on 3 dimensions that have already been mentioned in the literature: the Initial Approach, which shows that these workers usually initiate their practices with a family or an individual by going after them, seeking bond development with the service. The Specialized Follow-up, in which these workers develop their practices with diversity and creativity, through home visits and activities with group, family or individual, often with other workers, such as social assistance workers. And the Network Articulation, in which a significant movement toward working with the public services available on each territory is identified. Finally, there is still a path to be taken regarding the definition of psychological practices on CREAS, however, there is much to be noticed of what has already been occurring. Thus, academic research on such context is relevant since the inclusion of psychologists who work in this policy may promote an articulation between daily activity and the theoretical and academic reflection, supported by the research.(AU)


Este texto tuvo por objetivo explicar las acciones desarrolladas por psicólogas/os trabajadoras/es en los "Centros de Referência Especializados de Assistência Social" (CREAS) (Centros de Referencia Especializados en Asistencia Social), respecto a las situaciones de violencia intrafamiliar, identificadas por un estudio empírico hecho por una psicóloga-investigadora que actúa en un CREAS. Participaron 12 psicólogas/os que trabajan en estos centros, en siete ciudades del interior de Rio Grande do Sul (Brasil), donde se llevó a cabo las entrevistas en persona. El análisis de datos apuntó a una comprensión metodológica de tres dimensiones ya destacadas en la bibliografía: la Acogida Inicial, que enseña que estos profesionales generalmente empiezan sus prácticas con una familia o individuo buscando el encuentro para la promoción de la vinculación con el servicio; El Seguimiento Especializado, en el que desarrollan sus prácticas con diversidad y creatividad desde visitas domiciliarias, trabajos con grupos, individuos o familias, generalmente junto a otros profesionales, sobre todo con trabajadores sociales; y Articulación con la Red de Servicios, en la cual se identificó un importante movimiento para el trabajo con otros servicios disponibles en el territorio. Por fin, se observa que todavía hay un camino por recorrer en relación a la definición de las prácticas de psicólogos en CREAS, aunque hay mucho que decir respecto a las prácticas que ya están ocurriendo. Así, se vuelven relevantes las investigaciones académicas en ese contexto por introducir a los profesionales psicólogos trabajadores de la política, las cuales pueden promover una articulación entre la producción del hacer cotidiano y la reflexión teórica y académica sustentada por las investigaciones.(AU)

ABSTRACT

O objeto de estudo deste trabalho é a atuação de psicólogas(os) no campo da educação básica. Tivemos como objetivo investigar as práticas de atuação e os desafios enfrentados pelas(os) psicólogas(os) que trabalham na educação em Boa Vista (RR), com intuito de conhecer a inserção desses profissionais no sistema educacional. Trata-se de pesquisa qualitativa, orientada pelo referencial teórico-metodológico da Psicologia Escolar Crítica. Realizamos o processo de levantamento dos dados entre os meses de março e abril de 2018, por meio de entrevistas semiestruturadas, audiogravadas e transcritas. Encontramos 21 psicólogas e um psicólogo trabalhando em instituições educacionais e de ensino na cidade; a maioria atuava na educação básica; metade dos entrevistados ingressou por concurso público e a outra metade era contratada e comissionada; poucos(as) foram contratados(as) como psicólogos(as) escolares. Para a análise, selecionamos dez psicólogas(os) com mais tempo no cargo. Quanto às práticas de atuação, identificamos que 60% atuavam na modalidade clínica e 40% na modalidade clínica e institucional. Como desafios, encontramos melhoria das condições de trabalho; estabelecimento de relações hierárquicas e a dificuldade de fazer compreender as especificidades desse campo de trabalho; necessidade na melhoria das condições para formação continuada; atuação da(o) psicóloga(o) escolar enquanto ação institucional. Diante do exposto, compreendemos ser necessária uma mudança de paradigma na atuação das(os) psicólogas(os) que trabalham na educação na região, e a apropriação das discussões da área, principalmente, aquelas apresentadas pela Psicologia Escolar Crítica, vez que esta contribui para uma atuação que leve em conta os determinantes sociais, políticos, culturais e pedagógicos que constituem o processo de escolarização.(AU)


This work has as object of study the role of psychologists in the field of Basic Education. We aimed to investigate practices and challenges faced by psychologists who work in education in Boa Vista/RR, to know the insertion of these professionals in the educational system. This is a qualitative research, guided by the theoretical-methodological framework of Critical School Psychology. The data collection process was carried out between March and April 2018, with semi-structured, audio-recorded, and transcribed interviews. We found 21 female psychologists and one male psychologist working in educational institutions in the municipality; most worked in Basic Education; half of the interviewees had applied to work as government employee and the other half were hired and commissioned; few were hired as school psychologists. To carry out the analysis, we selected ten psychologists with more time in the position. Regarding the practices, we have identified that 60% worked in the clinical modality and 40% in the clinical and institutional modality. As challenges, we find the improvement in working conditions; the establishment of hierarchical relationships and the difficulty of making the specificities of this field of work understood; the need to improve conditions for continuing education; the practice of the school psychologist as institutional action. In view of the above, we understand that a paradigm shift in the performance of psychologists working in education in that region is necessary, and the appropriation of discussions in the area, especially those presented by Critical School Psychology, contributes to an action that considers social, political, cultural, and pedagogical determinants that constitute the schooling process.(AU)


Este trabajo tiene como objeto de estudio la actuación profesional de las(os) psicólogas(os) en la educación básica. Su objetivo es investigar las prácticas y retos que enfrentan las(os) psicólogas(os) que trabajan en la educación en Boa Vista, en Roraima (Brasil), con la intención de conocer la inserción de estos profesionales en el sistema educativo. Esta es una investigación cualitativa que se guía por el marco teórico-metodológico de la Psicología Escolar Crítica. Se recopilaron los datos entre los meses de marzo y abril de 2018, mediante entrevistas semiestructuradas, grabadas en audio y después transcritas. Las 21 psicólogas y un psicólogo trabajan en instituciones educativas de la ciudad; la mayoría trabajaba en la educación básica; la mitad de los entrevistados ingresaron mediante concurso público y la otra mitad era contratada y de puesto comisionado; pocos fueron contratados como psicólogos escolares. Para el análisis, se seleccionaron diez psicólogas(os) con más tiempo en el cargo. Con respecto a las prácticas, el 60% trabajaba en la modalidad clínica; y el 40%, en la modalidad clínica e institucional. Los retos son mejora de las condiciones laborales; establecimiento de relaciones jerárquicas y dificultad para comprender las especificidades de este campo de trabajo; necesidad de mejorar las condiciones para la educación continua; y actuación dela (del) psicóloga(o) escolar como acción institucional. En vista de lo anterior, es necesario el cambio de paradigma en la práctica profesional de las(os) psicólogas(os) que trabajan en la educación de la región y la apropiación de las discusiones del área, especialmente las presentadas por la Psicología Escolar Crítica, que contribuye a una acción que tiene en cuenta los determinantes sociales, políticos, culturales y pedagógicos que constituyen el proceso de escolarización.(AU)

ABSTRACT

Children under the age of five are prone to foodborne diseases, resulting from poor food hygiene because of their less effective immune systems. This study aimed to assess the knowledge, attitude and practice of food hygiene among nursing mothers attending under-five clinics.Method: This study was a descriptive cross-sectional study, involving 330 nursing mothers attending the under-five clinic in health care facilities. Data were collected using interviewer-administered questionnaires and analyzed using Statistical Package for Social Sciences (SPSS) version 21.A p-value < 0.05 at 95% confidence interval was considered statistically significant.Result: Eighty-two percent of the respondents had good knowledge of food hygiene; 95.2% had a good attitude and 82.7% demonstrated good practice. Knowledge on food hygiene was significantly associated with the age of the respondents (P=0.008) and their educational status (P=0.001), those with tertiary education doing better. Similarly, respondents with tertiary education had significantly better attitude (P=0.001 and practice (P=0.001). The predictors of knowledge on food hygiene were education status (odds ratio [OR] =0.249, 95% confidence interval [CI]: 0.126, 0.492) and occupation of the respondents (OR = 2.158, 95% CI: 1.156, 4.026). Educational status was also the predictor for positive attitude (OR =0.143, 95% CI: 0.031, 0.667) and practice of food hygiene (OR =0.394, 95% CI: 0.210, 0.738).Conclusion: The knowledge, attitude and practice of food hygiene among the respondents was good. Predictors of knowledge included level of education and occupation. Government at the State and Local Government levels should organize regular health promotion on food hygiene.

ABSTRACT

Polymyxin B, which is a last-line antibiotic for extensively drug-resistant Gram-negative bacterial infections, became available in China in Dec. 2017. As dose adjustments are based solely on clinical experience of risk toxicity, treatment failure, and emergence of resistance, there is an urgent clinical need to perform therapeutic drug monitoring (TDM) to optimize the use of polymyxin B. It is thus necessary to standardize operating procedures to ensure the accuracy of TDM and provide evidence for their rational use. We report a consensus on TDM guidelines for polymyxin B, as endorsed by the Infection and Chemotherapy Committee of the Shanghai Medical Association and the Therapeutic Drug Monitoring Committee of the Chinese Pharmacological Society. The consensus panel was composed of clinicians, pharmacists, and microbiologists from different provinces in China and Australia who made recommendations regarding target concentrations, sample collection, reporting, and explanation of TDM results. The guidelines provide the first-ever consensus on conducting TDM of polymyxin B, and are intended to guide optimal clinical use.

ABSTRACT

Las guías de práctica clínica han contribuido a resolver un problema al sintetizar la evidencia y ponerla al alcance del profesional sanitario, pero su desarrollo e implementación creciente en los últimos años ha dado lugar a nuevos inconvenientes que aún no han sido resueltos. En este artículo editorial, la autora repasa cuestiones no tenidas en cuenta por las guías de práctica clínica, incluso aquellas consideradas de buena calidad de acuerdo a los estándares actuales, y reflexiona en especial sobre el uso del tiempo de los médicos en la consulta, aspecto desatendido que atenta contra la sustentabilidad del modelo actual de cuidado propuesto por estas recomendaciones. Clinical practice guidelines have contributed to solving a problem by synthesizing the evidence and making it available to healthcare professionals, but their development and increasing implementation in recent years has given rise to new problems that have not yet been resolved. In this editorial article, the author reviews issues not taken into account by clinical practice guidelines, even those considered to be of good quality according to current standards, and reflects inparticular on the use of physicians' time in the consultation, a neglected aspect that undermines the sustainability of the current care model proposed by these recommendations. ABSTRACT

O Sistema de Avaliação de Testes Psicológicos (SATEPSI) recebeu notoriedade entre brasileiros e estrangeiros por oferecer um complexo sistema de qualificação dos testes psicológicos, pouco visto em âmbito mundial. Sua elaboração dependeu de uma autarquia, que o financiou, normatizou e o mantém, mas também de pesquisadores docentes de avaliação psicológica, que trouxeram a expertise da área para que houvesse o pleno estabelecimento de seus parâmetros. Passadas duas décadas de seu lançamento, o SATEPSI foi tema de artigos, capítulos, lives e diálogos digitais, nos quais foram destaque, de modo geral, as Resoluções do Conselho Federal de Psicologia, que o normatiza, e seus impactos para a área de avaliação psicológica - como, por exemplo, o aumento do número de pesquisas e de testes brasileiros qualificados. O que se pretende neste artigo é mencionar sua construção, à luz dos autores que vivenciaram o SATEPSI em funções e tempos distintos. Atenção especial será dada aos Métodos Projetivos, cuja história ainda é pouco revelada.(AU)


The system to evaluate psychological tests (Satepsi) received notoriety among Brazilians and foreigners for offering a complex system of qualification of psychological tests, which is rarely seen worldwide. Its development depended on an autarchy (which financed, standardized, and maintains it) and on researchers teaching psychological assessment, who brought their expertise to the area so its parameters could be fully established. After two decades of its launch, Satepsi was the subject of articles, chapters, lives, and digital dialogues, which usually highlighted the Resolutions of the Federal Council of Psychology that normatize psychological evaluation and their impacts, such as the increase in the number of qualified Brazilian tests. This study aims to mention its construction in the light of the authors who experienced Satepsi in different functions and times, giving special attention to Projective Methods, whose history remains to be shown.(AU)


El Sistema de Evaluación de Tests Psicológicos (SATEPSI) ganó notoriedad entre los brasileños y los extranjeros por ofrecer un complejo sistema de calificación de los tests psicológicos, poco frecuente a nivel mundial. Su elaboración dependió de una autarquía, que lo financió, lo estandarizó y lo mantiene, pero también de investigadores docentes de evaluación psicológica, que trajeron la experiencia del área para que hubiera el pleno establecimiento de sus parámetros. Tras dos décadas de su lanzamiento, SATEPSI fue tema de artículos, capítulos, en directo y diálogos digitales, en los cuales destacaron, de modo general, las Resoluciones del Consejo Federal de Psicología que lo normatiza y sus impactos para el área de evaluación psicológica, como el aumento del número de investigaciones y de pruebas brasileñas calificadas. Lo que se pretende en este artículo es mencionar su construcción, a la luz de los autores que vivieron el SATEPSI en funciones y tiempos distintos. Se prestará especial atención a los métodos proyectivos cuya historia aún no se ha revelado.(AU)

ABSTRACT

A Comissão Consultiva em Avaliação Psicológica (CCAP), atrelada ao Sistema de Avaliação de Testes Psicológicos (SATEPSI) do Conselho Federal de Psicologia (CFP), tem como objetivos emitir pareceres acerca de solicitações advindas da avaliação psicológica(AP), elaborar e propor atualizações de documentos técnicos e normativos do CFP relativos à AP, elaborar e propor diretrizes para o ensino e formação continuada em AP, conduzir o processo de avaliação dos instrumentos submetidos ao SATEPSI e discutir temas e propor ações no âmbito da AP. Nos últimos 20 anos, a CCAP vem buscando atender a esses objetivos, indicando novos caminhos para a área. Nesse sentido, este artigo tem como objetivo apresentar as principais atualidades e movimentos da CCAP, indicando caminhos possíveis e perspectivas futuras para a área de AP. São discutidas as ações atuais que vêm sendo desenvolvidas pela CCAP, bem como as ações futuras delineadas que buscam promover uma AP cada vez mais democrática. Concluímos que a AP é uma prática do(a) psicólogo(a) que deve ser operacionalizada com compromisso ético, atrelada aos direitos humanos e à justiça, com embasamento científico e alinhada às mudanças sociais.(AU)


The Consultative Commission on Psychological Assessment (CCAP), affiliated with the Psychological Test Evaluation System under the Federal Council of Psychology (CFP), has the following objectives: to provide expert opinions on requests stemming from psychological assessments (PA), to draft and propose updates to the CFP technical and normative documents pertaining to PA, to formulate and recommend guidelines for education and ongoing professional development in PA, to oversee the evaluation process of instruments submitted to SATEPSI, and to engage in discussions and propose initiatives within the PA. Over the past two decades, CCAP has diligently worked to achieve these goals, charting new avenues in the field. In this context, this study aims to describe the most current developments and initiatives of CCAP and outline prospective directions and future outlooks for the PA. This study delves into the current initiatives undertaken by CCAP and the prospective actions delineated to foster a progressively more inclusive PA. Thus, we claim that PA is a practice inherent to psychologists that demands ethical commitment, alignment with human rights and justice, a solid scientific foundation, and adaptation to evolving social dynamics.(AU)


La Comisión Consultiva en Evaluación Psicológica (CCAP), vinculada al Sistema de Evaluación de Pruebas Psicológicas (SATEPSI) del Consejo Federal de Psicología (CFP), tiene como objetivo emitir opinión técnica sobre solicitudes derivadas de la evaluación psicológica (EP), elaborar y proponer actualizaciones de documentos técnicos y normativos del CFP relacionados con EP, desarrollar y proponer lineamientos para la enseñanza y la formación continua en EP, conducir el proceso de evaluación de los instrumentos presentados al SATEPSI y discutir temas y proponer acciones en el ámbito de EP. Durante los últimos veinte años, la CCAP ha buscado alcanzar estos objetivos indicando nuevos caminos para el área. En este sentido, este artículo tiene como objetivo presentar las principales actualidades y movimientos de la CCAP indicando posibles caminos y perspectivas de futuro para el área de EP. Se discuten las acciones actuales que ha desarrollado la CCAP, así como las acciones futuras perfiladas que buscan promover una EP cada vez más democrática. Se concluye que la EP es una práctica del psicólogo que debe ponerse en práctica con compromiso ético, vinculada a los derechos humanos y la justicia, con base científica y alineada con los cambios sociales.(AU)

BACKGROUND@#This study aimed to develop a comprehensive instrument for evaluating and ranking clinical practice guidelines, named Scientific, Transparent and Applicable Rankings tool (STAR), and test its reliability, validity, and usability.@*METHODS@#This study set up a multidisciplinary working group including guideline methodologists, statisticians, journal editors, clinicians, and other experts. Scoping review, Delphi methods, and hierarchical analysis were used to develop the STAR tool. We evaluated the instrument's intrinsic and interrater reliability, content and criterion validity, and usability.@*RESULTS@#STAR contained 39 items grouped into 11 domains. The mean intrinsic reliability of the domains, indicated by Cronbach's α coefficient, was 0.588 (95% confidence interval [CI]: 0.414, 0.762). Interrater reliability as assessed with Cohen's kappa coefficient was 0.774 (95% CI: 0.740, 0.807) for methodological evaluators and 0.618 (95% CI: 0.587, 0.648) for clinical evaluators. The overall content validity index was 0.905. Pearson's r correlation for criterion validity was 0.885 (95% CI: 0.804, 0.932). The mean usability score of the items was 4.6 and the median time spent to evaluate each guideline was 20 min.@*CONCLUSION@#The instrument performed well in terms of reliability, validity, and efficiency, and can be used for comprehensively evaluating and ranking guidelines.

ABSTRACT

American Academy of Orthopaedic Surgeons (AAOS) just released the up-to-date <Management of Hip Fractures in Older Adults:Evidence-Based Clinical Practice Guideline>, which has become the principles to care hip fractures in the elderly. In comparison to the Guideline 2014, considerable changes are made in terms of guideline composition and focused items. The interval of 7 years yielded dramatic progress in the care of geriatric hip fractures, including the recommendation of cemented femoral stems in hip arthroplasty due to displaced femoral neck fractures, cephalomedullary device for unstable intertrochanteric fractures and tranexamic acid to reduce blood loss and blood transfusion. Additionally, the individualized properties of the elderly with hip fractures should be noted to balance an early operation within 24 and 48 hours and patient safety. The interpretation of <Management of Hip Fractures in Older Adults:Evidence-Based Clinical Practice Guideline> is helpful to comprehensively understand the progress of the care of geriatric hip fractures, thus to make orthopaedic surgeons master the key points of clinical practice, and to improve the quality of operations and decrease perioperative complications.
